For one off changes outside a plan and for bigger jobs like new pages, I quote first so you know the cost before any work starts. There are no surprise invoices for a ten minute fix.
If your site has so many outdated parts that patching them is not worth it, I will tell you. Sometimes a website redesign works out better value than a long list of fixes.
